OFFICE VISIT - Dr. Jennings sees the Plaintiff, Robert Cooney, for a second
opinion as to his right hip prosthesis. |

HISTORY - Plaintiff indicates to Dr. Jennings that he fell on 3/2/01 onto a
sidewalk and sustained a fracture of his femoral neck. He underwent a bipolar
prosthetic replacement of his hip and then spent forty days in a rehabilitation center.
He states that he did well after two months and returned to work as a machinist
in May, 2001. |

CHIEF COMPLAINT - Plaintiff indicates that recently he has had progressive
pain and is unable to put on his shoes, "stockings" (support hose for his edema on
right leg and foot as mentioned in Dr. William's report of 5/28/01) or cut his toenails.
He also has difficulty getting in and out of a car. He denies any medication. On
this date, he complains of some pain on forced internal rotation of the right hip. |

FINDINGS - Loose right hip prosthesis is ruled out and he is scheduled for a total
body bone scan. |

OFFICE VISIT - Dr. Jennings indicates that Plaintiff had a bone scan done at
Palmetto General Hospital after his last visit to this office on 3/30/01. (He states that
the report is attached, however, there is no report in the records sent to us.) He
states that the reading radiologist felt that the report showed "no specific
abnormalities." However, Dr. Jennings disagrees. He believes there is increased
activity in the right acetabulum. |

OFFICE VISIT The Plaintiff complains of pain on forced internal rotation on this
date, and Dr. Jennings explained to him that he believes the pain is coming from his
hip. He informs Plaintiff that he could either "tolerate it or consider revision
surgery." The Plaintiff states he will consider this and will let the doctor know. |
